I consent to Made For You Co providing the newborn hand and/or foot casting service for my baby.*

I understand that the service involves creating a temporary mould of my baby's hand and/or foot using the casting materials specified for the service, followed by the creation and finishing of a permanent keepsake cast.*

I understand that the casting process is carried out with my baby under my care and supervision and that I remain responsible for supporting and supervising my baby throughout the appointment.*

I understand that Made For You Co may pause or stop the casting process at any time if my baby becomes distressed, unsettled or unable to safely participate.

I understand that I may request that the casting process be paused or stopped at any time.*

I understand that Made For You Co may decline to proceed with or discontinue a casting where, in its reasonable judgement, continuing would not be appropriate or safe.*

I understand that hand and foot casts are individually handmade and that minor variations, markings, surface characteristics and imperfections may occur as part of the casting and finishing process.*

I understand that the final cast may not be an exact representation of every detail of my baby's hand or foot.*

I understand that, on occasion, a mould or finished cast may not achieve a suitable result and that Made For You Co may need to remake the cast where reasonably possible.*

I understand that Made For You Co will discuss any significant issue affecting the final product with me.*
