I consent to photos/video of my baby's casting being used on Made For You Co's social media.

I consent to photos of the finished casting being used on the Made For You Co website and online portfolio.

I consent to photos/video being used in Made For You Co advertising and promotional materials.

I consent to my testimonial/review being used for marketing purposes.

I understand that I can decline any or all marketing permissions and this will not affect my ability to receive the casting service.
